8-1-1: COMPLIANCE REQUIRED:
   A.   It shall be unlawful in the Village of Oak Park for any person, either directly or indirectly, to conduct any business or nonprofit enterprise, or to use in connection therewith any vehicle, premises, machine or device regulated by this chapter or for which a license is required by any other chapter of this code, or for which a license fee is established in article 2 of this chapter, without first having obtained a license to do so.
   B.   Contractors who seek to perform work for which a permit is required must register with the Village in accordance with chapter 7, article 1 of this code, regardless of whether they maintain a fixed place of business in Oak Park.
   C.   Whenever a provision in this chapter refers to "the Village Clerk" or "the Village", it shall mean "the Village Manager or the Manager's designee". (Ord. 2014-0-52, 8-4-2014)
8-1-2: APPLICATIONS:
Applications for all licenses shall be made in writing to the Village Manager or the Manager's designee on forms provided by the Manager or the Manager's designee.
Each application shall state:
   A.   The name of the applicant;
   B.   Whether the applicant is an individual, partnership or corporation:
      1.   If an individual, the applicant shall provide the name and the residence (and any business) address of the applicant;
      2.   If a partnership, the applicant shall provide the business and residence address of each partner, including limited partners;
      3.   If a corporation, the applicant shall provide the name, state of incorporation, and the names and business and residence addresses of the officers, directors and managers in charge of any local facility thereof;
   C.   The permit or license desired;
   D.   The location and mailing address of the business, the telephone number(s) of the business, and an e-mail address for the owner or general manager;
   E.   Whether the premises are owned or leased by the applicant; and if leased, the name and address of the lessor;
   F.   The number of square feet of space to be occupied by the business;
   G.   The business or activity to be licensed;
   H.   The date the license was applied for and if the proposed business is temporary, the time period for which the license is requested;
   I.   The number of employees or agents;
   J.   Whether the business is a "food establishment" as defined in section 8-24-1 of this chapter;
   K.   Whether there are or will be any coin operated devices on the property and if so, the name, address and telephone number of the owner of the coin operated devices.
Each application shall contain such additional information as may be required or may be necessary to provide for the proper guidance of Village officials in the issuance of the license. (Ord. 2014-0-52, 8-4-2014)
8-1-3: PERSONS SUBJECT TO LICENSE:
Whenever in this code a license is required for the maintenance, operation or conduct of any business or establishment, or for doing business or engaging in any activity or occupation, every person shall be subject to the requirements if, by himself or through an agent or employee, he holds himself forth as being engaged in the business or occupation; or solicits patronage therefor; or performs or attempts to perform any such business or occupation in the Village.
