Applications for any examination shall be made upon application forms prepared and approved by the Commission. All applications shall be legibly printed or written and applicants shall state under oath or affirmation all pertinent facts which may include the following subjects:
a. Name, address and telephone number.
b. Military Service.
c. Skills.
d. Previous employment.
e. Education.
f. References.
g. Such other information affecting the qualification of the applicant for admission to the examination as may be required of all such applicants by the Commission.
