A certificate of occupancy issued for a building or structure pursuant to the provisions of this chapter shall be in a form prescribed by the building official and contain the following information:
A. The address of the building or structure;
B. The name and address of the owner of the building or structure;
C. A description of any portion of the building or structure for which the certificate is issued;
D. A statement that the building or structure or any portion thereof for which the certificate is issued complies with the building standards adopted by Chapter 16.06 of this title which are applicable to the particular group and division of occupancy or use for which the building or use for which the building or structure or portion thereof is to be classified.
E. The number of the building permit, if any, that was issued for the building or structure.
