Sec. 4-114 Ambulance Service Rates.
   The Town of Schererville, Lake County, Indiana, shall charge the following rates for ambulance emergency medical services and related provided by the Town of Schererville Metropolitan Fire Department Ambulance Service commencing on August 1, 2021:
   a.   Basic Life Support (BLS) Transport Fees.
      1.   Residents of the Town of Schererville, Lake County, Indiana, receiving ambulance and related emergency medical services, which services include transport, shall be charged the amount of seven hundred sixty-eight dollars ($768.00) per call, and shall be charged twenty-four and 50/100 dollars ($24.50) per mile traveled.
      2.   Non-residents receiving ambulance and related emergency medical services, which services include transport, shall be charged the amount of one thousand one hundred nine dollars ($1,109.00) per call, and shall be charged twenty-four and 50/100 dollars ($24.50) per mile traveled.
      3.   Residents of the Town of Schererville, Lake County, Indiana, receiving ambulance and related emergency medical services consisting of treatment and site release shall be charged the amount of two hundred fifty-six dollars ($256.00) per call. The rates and charges herein for treatment and site release shall be inclusive of lift assistance in any care facility or residential home wherein request was made and service response of the Town Metropolitan Fire Department occurred. This user fee/rate charge shall be made to each of the owner/operator of the care facility, or the owner/occupant of the residential property. The user fees charged for such lift assistance and care at a care facility or at a residential property shall be charged, and upon collection, such rates and charges shall be deposited for Fire Department use in the Town Special Ambulance Equipment Non-Reverting Fund, with use thereafter to be in conformance with the rules, regulations and requirements of the Indiana State Board of Accounts.
      4.   Non-residents receiving ambulance and related emergency medical services consisting of treatment and site release shall be charged the amount of four hundred twenty-seven dollars ($427.00) per call. The rates and charges herein for treatment and site release shall be inclusive of lift assistance in any care facility or residential home wherein request was made and service response of the Town Metropolitan Fire Department occurred. This user fee/rate charge shall be made to each of the owner/operator of the care facility, or the owner/occupant of the residential property. The user fees charged for such lift assistance and care at a care facility or at a residential property shall be charged, and upon collection, such rates and charges shall be deposited for Fire Department use in the Town Special Ambulance Equipment Non-Reverting Fund, with use thereafter to be in conformance with the rules, regulations and requirements of the Indiana State Board of Accounts.
   b.   Advance Life Support (ALS, Level 1) Transport Fees. For the Advance Life Support, Level 1, Level of Service, the same shall be defined as set forth and provided hereinafter as described, namely:
      Advance Life Support (ALS), Level 1. Advance Life Support Level 1 (ALS1) is the transportation by ground ambulance vehicle and the provision of medical necessary supplies and services including the provision of an ALS assessment of at least one (1) ALS intervention.
      Advanced Life Support Assessment. Advance life support (ALS) assessment is an assessment performed by an ALS crew as part of an emergency response that was necessary because the patient's reported condition at the time of dispatch was such that only an ALS crew was qualified to perform the assessment. An ALS assessment does not necessarily result in determining that the patient required an ALS level of service.
      The determination to respond with an ALS ambulance must be in accord with the local 911 or equivalent service dispatch protocol.
      1.   Residents of the Town of Schererville, Lake County, Indiana, receiving ambulance and related emergency medical services, which services included transport, shall be charged the amount of one thousand one hundred ninety-five dollars ($1,195.00) per call, and shall be charged twenty-four and 50/100 dollars ($24.50) per mile traveled.
      2.   Non-residents receiving ambulance and related emergency medical services, which services include transport, shall be charged the amount of one thousand four hundred fifty-one dollars ($1,451.00), and shall be charged twenty-four and 50/100 dollars ($24.50) per mile traveled.
   c.   Advance Life Support (ALS), Level 2 Transport Fees. For the Advance Life Support Level 2 of service, the same shall be defined as set forth and provided hereinafter as set forth and provided as described, namely:
      Advance Life Support (ALS), Level 2. Advance life support, Level 2 (ALS2), is the transportation by ground ambulance vehicle and the provision of medically necessary supplies and services including: (A) at least three (3) separate administrations of one (1) or more medications by intravenous push/bolus or by continuous infusion (excluding crystalloid fluids); or (B) ground ambulance transport and the provision of at least one (1) of the ALS2 recognized and identified procedures.
      1.   Residents of the Town of Schererville, Lake County, Indiana, receiving ambulance services, which services include transport and are identified as ALS Level 2, shall be charged the amount of one thousand two hundred eighty dollars($1,280.00) per call, and shall be charged twenty four and 50/100 dollars ($24.50) per mile traveled.
      2.   Non-residents receiving ambulance and related emergency medical services consisting of treatment and site release shall be charged the amount of one thousand five hundred seventy-nine dollars ($1,579.00) per call, and shall be charged twenty-four and 50/100 dollars ($24.50) per mile traveled.
