Sec. 4-85 Application.
   An application for the license required by this Division containing the following information shall be furnished the Clerk-Treasurer two (2) weeks prior to the issuance of the license:
   a.   The applicant's name, home address and local address, if any;
   b.   A photograph and physical description of the applicant, setting forth the applicant's age, sex, height, weight, date of birth, social security number, race, complexion, color of hair and eyes and other distinguishing features;
   c.   The fingerprints of the applicant and the names of at least two reliable property owners of the county of Lake, State of Indiana, who will certify as to the applicant's good character and business responsibility, or, in lieu thereof, other evidence as to good character, sufficient to make an evaluation as to character and responsibility;
   d.   The name and address or principal office of the person, if any, for or through whom or under those auspices the applicant is authorized to conduct such activity, or a statement that the applicant is engaged in such activity solely on his own behalf;
   e.   A brief description of the type of goods, wares or merchandise to be sold and a statement whether delivery of such goods, wares or merchandise is to be made immediately or in the future;
   f.   Each applicant dealing in an edible product or traveling from house to house shall file with his application a statement by a reputable physician of the Town of Schererville, dated not more than ten (10) days prior to submission of the application, certifying the applicant and goods to be free of infectious, contagious, or communicable disease, or contaminants;
   g.   A statement as to whether or not the applicant has been convicted of any crime or misdemeanor and, if so, the nature of the offense and the punishment assessed therefore, the date and location and Court of such conviction;
   h.   If the applicant canvasser or solicits orders for such wares, goods or merchandise, a sample copy of the order or receipt form used by him in connection with such activity;
   i.    The length of time for which the right to do business is desired;
   j.    If a vehicle is to be used, a description of same, together with license number or other means of identification.
