All ordinance violations included within this ordinance shall be filed on a form entitled "Town Of Schererville Ordinance Violation Citation which shall state:
a. That the Town of Schererville, Indiana, a municipal corporation, is the Plaintiff;
b. The name, address, telephone number, Social Security Number and date of birth of the Defendant, if known;
c. The date and time when the Ordinance violation occurred;
d. The specific Ordinance alleged to have been violated by the Defendant;
e. The place where the Ordinance violation occurred;
f. The manner in which the Ordinance was violated;
g. Whether the violation is a single occurrence or a continuing violation. If the violation is continuing, the date such violation commenced;
h. The signature of the complainant, affirming under oath that he has reasonable and just grounds to believe that the defendant violated the Ordinance;
i. The approval of the Town Attorney or his designate;
j. If the violation is a single occurrence, that it is payable and that the defendant has ten (10) days to pay the designated fine in the Office of the Clerk-Treasurer of the Town of Schererville, and that if the fine is not paid within said period of time, that the defendant will be cited to appear in Court.
