Application for inclusion on a rotation list shall be for a period of one year, from July 1 through June 30, or any portion thereof. Application made by June 1 shall be eligible for inclusion as of the following July1. Applications submitted at another time shall be eligible when the Mayor has approved the application and will not interrupt any current published rotation schedule. Application shall be made upon blank forms prepared and made available by the Mayor and shall state:
(A) The name, home address, home phone, business phone, and proposed business address of the applicant.
(B) The type of ownership of the business, i.e., corporation, limited liability company, sole proprietorship, etc., all officers and agents of the business, and affirm that the applicant is the legal entity that will be owning and operating the wrecker business.
(C) The location and description of the towing equipment owned or leased only by the applicant. If such equipment is not owned by the applicant, a copy of the lease must be included.
(D) That the applicant has available space for properly accommodating and protecting all motor vehicles to be towed, as required under section 7-7-8 of this ordinance.
(E) Such other information as the Mayor shall find reasonably necessary to effectuate the purpose of this article and to arrive at a fair determination of whether the terms of this article have been complied with.
(F) That the applicant is the owner of a Warrant, Tax Clearance Certificate from the New Mexico Taxation and Revenue Department, Certificate of Good Standing from the Corporations Bureau of the New Mexico Public Regulation Commission, copy of the Public Regulation Commission's Department of Transportation's vehicle inspection reports, and has registered with the City of Albuquerque. A copy of these Certificates and vehicle inspection reports shall be included with the application.
(G) If the application is approved, and if any of the information contained therein changes, the application shall be updated by the applicant within 10 calendar days of such change. Failure to comply with this provision shall be grounds for removal from the rotation list.
