(a) Commencing in calendar year 2004, each full-time employee shall be entitled to and will receive longevity pay under the following schedule:

(b) Longevity pay shall be computed only on years of full-time employment with the City. The City shall have the option of making such payment by separate check or identifying the payment as a separate item. Longevity shall be paid in the month of June based upon years of service as of December 31 of the applicable year.

(a) Any full-time employee within the Department of Fire who has obtained an Associate's Degree in fire science shall receive six hundred fifty dollars ($650.00) additional compensation annually, prorated in the year of completion. This compensation shall be paid in the first pay period of December.
(b) Any full-time employee within the Department of Fire who has obtained a Bachelor's Degree in fire science or in a field related to the position held by the employee as approved by the Director of Safety and the Mayor shall receive one thousand dollars ($1,000.00) additional compensation annually, prorated in the year of completion. This compensation shall be paid in the first pay period of December.

(a) Each newly hired full-time firefighter/paramedic shall, during the first year of employment, be provided with uniform items and turnout gear as determined appropriate by the Chief of Fire. Employees advanced to a full-time position in calendar year 2003 shall be provided uniform items and turnout gear as determined appropriate.
(b) Effective January 1, 2004, each full-time employee of the Fire Department, excluding dispatchers, shall after the first year of full-time employment, receive a yearly uniform allowance of seven hundred fifty dollars ($750.00). Each full-time dispatcher shall receive a yearly uniform allowance of four hundred fifty dollars ($450.00). Yearly uniform allowances will be issued with one-half paid in April and one-half paid in September.
(c) (1) In the event of damage to uniform items, including watches and prescription eyeglasses, which damage occurs in the active discharge of the employee's duties suppressing fires or performing emergency medical procedures, the City shall pay the difference between the amount of reimbursement from the employee's personal insurance, court ordered recovery and/or workers' compensation, and the cost of repair or replacement at the current fair market value. The City's monetary reimbursement obligation for repair or replacement of the following articles shall not exceed the amounts set forth below:
A. Watch $30.00
B. Prescription eyeglasses $100.00
C. Uniform jacket $200.00
(2) Line of duty damage shall be reported immediately to the Chief of Fire. Upon approval of the Chief of Fire and the Director of Safety, the above-noted payment shall be forwarded to the Department of Finance for processing of payment to the employee. Workers' compensation claims will be filed by the employee where applicable. In the event the City reimburses the employee prior to the employee's receipt of personal insurance, court ordered recovery, and/or workers' compensation, reimbursement to the City will be made upon receipt of any monetary award by the employee.

The City will pay the monthly premium for accidental death, dismemberment, loss of sight, and permanent disability insurance to the Ohio Municipal League Group Accident Plan. Additional amounts of insurance may be purchased at no cost to the City, with the yearly premium payable in advance. This benefit shall apply to all full-time employees.

(a) The City shall provide eligible employees with the same medical insurance (health plan) as provided to other City employees under a group insurance plan. Such group insurance may be provided through a self-insured plan or an outside provider. Cost containment measures may be adopted by the City in consideration of projected costs, marked availability of coverages and utilization.
(b) The City agrees to pay 80% of the premium/contribution costs for health coverage for each eligible full-time employee enrolled in any of the health coverage plan offered by the City. The election of single or family coverage rests with the eligible employee. Any premium/contribution cost in excess of the 80% established herein shall be the responsibility of the eligible employee.
(c) Any eligible full-time employee may waive health plan coverage and be paid one hundred twenty dollars ($120.00) per month in lieu of such coverage. The waiver must be requested in writing, from the Finance Director thirty days prior to the beginning of any billing cycle. Applicable waiver amounts shall be payable by the City to the eligible employees in June and December of each year. Employees who have waived health plan coverage may enroll in the health plan pursuant to the requirements of the County. Upon re-enrollment, such employees shall forfeit the waiver.
(d) The City, at its sole cost and expense, shall provide each full-time employee with group life insurance coverage in the face amount of twenty-five thousand dollars ($25,000). Employees aged 65 years and over may receive a lowered face amount, depending on the carrier's benefits schedule.
(e) Where an employee is on personal sick leave, his or her medical insurance shall be continued for a period that he or she receives payments for accumulated sick leave and vacation time. Where an employee continues to be disabled, due to a line-of-duty illness or injury, and after using all accumulated sick leave and vacation time, medical insurance costs as provided in this section will be paid by the City for up to 6 additional months of disability. Upon exhaustion of those benefits, medical insurance may be continued as provided by statute, at the option of the employee, by his or her paying the full premium cost on a monthly basis directly to the City.
(f) Part-time employees, elected officials and former employees eligible under the Consolidated Omnibus Budget Reconciliation Act (COBRA) may participate in the insurance program. However, this shall be at no cost to the City, and discretion with reference to such employees is given to the Finance Director.
