Application for such license shall contain the following information:
(a) Name and address of the applicant, age, date and place of birth.
(b) Prior conviction of applicant, if any.
(c) Place where juke box is to be displayed or operated and the business conducted at that place.
(d) Description of juke box to be covered by the license, mechanical features, name of manufacturer and serial number.
No license shall be issued to any applicant unless he shall be over twenty-one years of age and a citizen of the United States.

Application for license shall be made out in duplicate, one copy being referred to the Chief of Police and the other copy to the City Electrical Inspector.
(a) The Chief of Police shall investigate the location wherein it is proposed to operate such juke box, ascertain if the applicant is a person of good moral character and either approve or disapprove the application.
(b) No license shall be issued to any applicant unless approved by the Chief of Police and the Electrical Inspector.

Every applicant, before being granted a license, shall pay to the Mayor a license fee for the privilege of operating or maintaining for operation juke boxes, as defined in Section 731.01, the sum of twenty dollars ($20.00) per juke box. Each license shall expire on December 31 next following the date of issuance.

(a) The license herein provided for shall be posted permanently and conspicuously at the location of the juke box in the premises wherein the same is to be operated or maintained to be operated.
(b) Such license maybe transferred from one juke box to another upon application to the Mayor to such effect and the giving of a description and the serial number of the new juke box. Not more than one juke box shall be operated under one license and the applicant for license shall be required to secure a license for each and every juke box displayed or operated by him.
(c) If the licensee shall move his place of business to another location within the City, the license may be transferred to such new location upon application to the Mayor, giving the street and number of the new location. The new location shall be approved by the Chief of Police and the City Electrical Inspector in the same manner as provided in Section 731.06.

(a) No person holding a license under this chapter shall permit the playing of juke boxes between the hours of 3:00 a.m. and 8:00 a.m. of any day.
(b) No person shall permit the playing of juke boxes within two hundred feet of any church, public or parochial school or playground.
