Any person, firm or corporation using a taxicab or motor vehicle for hire, as mentioned in this chapter, shall file his application in writing with the city clerk showing:
A. The name of the owner;
B. The name of the operator of such vehicle;
C. The purpose for which such vehicle is offered to or used by the public;
D. The serial number or engine number of each such motor vehicle; and
E. A description of such vehicle by make and model. (2000 Code § 16-203)
