63-1-4: APPLICATION PROCESS:
Application must be made to the City's Engineering Division. Applications should include the following:
   A.   Insurance Certificate: Certificate of insurance, naming Salt Lake City Corporation as an additional insured ($2,000,000.00 general aggregate and $1,000,000.00 per occurrence).
   B.   Site Plan: Site plan, including property lines, sidewalk widths, tree locations, fire hydrants, streetlight locations, vaults in the right-of-way, adjacent business access, location and number of chairs and tables, and any other information as required by the Engineering Division.
