(a) General. Application shall be prepared by the property owner or the authorized agent, architect, engineer or contractor employed by the owner in connection with the proposed work. Application shall be signed by the property owner and notarized by an authorized agent.
(b) Form of Application. Application shall be prepared in writing on forms prescribed by the Director of Building and Housing and shall contain the full names and addresses of the owner, applicant and contractor, the street and plat number of the parcel(s), a detailed description of the proposed work with relevant ASMT and/or AASHTO standards to be utilized, the use and occupancy(ies) for which the proposed work is intended, the estimated cost thereof a statement in behalf of the owner that the proposed work and occupancy will be in accordance with all laws and ordinances applying to the premises and such other information as may be required by the Director. Applications shall be signed and notarized by the property owner and electronically filed applications shall bear a facsimile signature and indicate the authority of the person affixing the facsimile to the application.
(c) Construction Documents. A minimum of three (3) sets of construction documents are required to be submitted with the application for a permit to excavate or fill land and shall be prepared by a registered professional engineer or an architect where the work contemplated is incident to the practice of architecture and in addition to the ordinance requirements. Plans shall be drawn on paper or other medium acceptable to the Director of Building and Housing, drawn to scale and shall be sufficiently detailed and legible so that, together with the accompanying specifications and other data, any person who is technically competent can determine whether or not the proposed grading operation will conform to the standards outlined in this chapter.
The plans and documents shall include the following:
(1) A contour map of the affected area showing the existing and proposed contours at five (5) foot intervals.
(2) The estimated amount of excavation or fill in cubic yards.
(3) The location of any existing and proposed buildings or structures on the site and within fifteen (15) feet of the site.
(4) The proposed method for the protection of soils from erosion.
(5) A signed and stamped report from a licensed design professional showing the results of surface and sub-surface exploration, conditions of the land, and procedures for performing the operation. A structural loading analysis, signed and stamped by a structural engineer, for underground sewer and culvert pipes under the proposed fill will also be necessary for review.
(6) All drainage provisions which shall be of such design to adequately handle the surface runoff, together with a map showing the drainage area of all land tributary to the site, the proposed disposition of surface drainage, and the estimated cubic foot per second runoff of the area served by any drain computed in accordance with current acceptable standards.
(7) The method to be used for and the degree of its proposed compaction. If vibratory or dynamic compaction methods are utilized, the provisions for minimizing such effects to adjacent and neighboring properties and buildings within fifteen (15) feet of the site will also be detailed.
(8) Proposed preparation of existing ground surface to receive fill.
(9) Proposed sub-surface drainage if necessary for stability.
(10) Plans for all retaining walls, cribbing, vegetative provisions, erosions and sediment control measures, together with location of temporary and/or permanent fencing and other protective devices to be constructed in connection, or as part of the proposed work.
(11) Clearly defined and technical requirements of the material to be used for the fill, including notation relative to the moisture content, soil type and plasticity pursuant to current AASHTO and/or ASTM guidelines.
(12) Verification that all materials come from approvable processing, borrow or manufacturing sites and an assertion that all other state, federal and local requirements are met for the removal, transportation and processing of said materials.
(13) Excavation or Fill Barricades. When deemed necessary for public safety by the Director of Building and Housing, for temporary or permanent excavation and fill operations not otherwise protected, the construction of a substantial barricade or fence not less than six (6) feet in height enclosing the excavated or filled construction area will be required.
