3-1-1: APPLICABILITY OF PROVISIONS:
All business, commercial and industrial activities in the village of whatever nature except those listed in section 3-1A-2 of this chapter are required to obtain a license to operate. No person, firm or corporation not otherwise licensed by this code except those engaged in a business listed in section 3-1A-2 of this chapter shall engage in any commercial, business or industrial activity of whatever kind in the village without first having obtained a business license as provided in this chapter. Application for a business license shall be made in conformance with the requirements of section 3-1-2 of this chapter relating to application for licenses. (Ord. 2003-02, 3-3-2003)
3-1-2: APPLICATION FOR LICENSE OR CERTIFICATE OF REGISTRATION:
Each application for a license or a certificate of registration shall state, at a minimum, the following:
   A.   Whether the application is for a new business, a change of ownership, or a change of business location;
   B.   The name, home address, telephone number, social security number, driver's license number, date of birth, fax number, and e-mail address of the applicant as well as his relationship to the business;
   C.   The business name, name it is doing business as (if different), type of business, nature of business, business address, telephone number, FEIN number, state sales tax number, present zoning of property where business is to be located, and whether the business is a permitted or special use;
   D.   Whether changes will be made to the interior or exterior of the business premises;
   E.   For the premises of the building, the square footage of the interior, the number of employees, the hours of operation, the number of delivery or service vehicles, and whether any flammable or hazardous material will be used or stored;
   F.   For each owner, partner or officer of the business, his or her name, title/position, home address, telephone number, social security number, driver's license number, and date of birth;
   G.   If the business is a corporation, the corporation name, address, telephone number, and name of contact;
   H.   If the property owner is different from the applicant, the property owner's name, address, telephone number, and, if applicable, a copy of a lease agreement;
   I.   Whether a liquor license, vending license, or other specialty license will be applied for; and
   J.   Any other information required by the village in order for them to administer the provisions of this code. (Ord. 2003-02, 3-3-2003)
