4-325: SUMMONS AND COMPLAINT:
   A.   Any person who witnesses or has personal knowledge that an act or acts made unlawful by this chapter have been committed may sign a complaint against the alleged violator.
   B.   Any police officer, animal control officer or Code Enforcement Officer who is employed by the City is authorized to issue a complaint when said officer personally observes a violation of this chapter.
   C.   The complainant must provide a sworn complaint to the officer receiving the complaint containing the following information:
      1.   Name, address and telephone number of the complainant and other witnesses to the incident.
      2.   Date, time and location of the incident.
      3.   Description of the animal.
      4.   Name, address and telephone number (if known) of the animal owner.
      5.   A statement that the animal attacked the complainant or some other person or animal as witnessed by the complainant, or such facts as warrant a finding that the animal is vicious, dangerous or diseased.
      6.   Other facts and circumstances of the incident. (Ord. 307, 7-9-2015)
