A. Application To City; Forms: A license required by this chapter may be obtained by making proper application to the city on forms furnished by the city. (2000 Code § 9-204)
B. Contents Of Application: Such application shall contain the following information for each applicant and each employee operating within the corporate limits of the city: (2000 Code § 9-204; amd. Ord. 1073, 3-11-2008, eff. 4-10-2008)
1. The name, address and telephone number of the applicant; if a post office box is given, also specify the permanent street address of the applicant.
2. Provide social security number, valid driver's license number and photo identification of applicant.
3. Current OTC resale or use tax number or similar business tax identification number from another state.
4. Name, address and telephone number of all suppliers of goods to be sold.
5. Any history of consumer complaints regarding the product to be sold, warranties attaching thereto, or business practices; specify the city and state where such complaint was registered.
6. Specify any licenses held and bonds in force.
7. List all names used by applicant or the company applicant represents within the last two (2) years, list the last three (3) cities wherein the applicant conducted business, and provide the address of the business location in those cities. (2000 Code § 9-204)
