727.02 LICENSE REQUIRED.
No person, firm, partnership, association or corporation shall provide or offer to provide a health spa service or engage in the business of health spa service by means of a prepaid health spa service contract without first having obtained a license therefor from the Health Department. 727.03 LICENSE APPLICATION; FEE.
The Health Department may grant a health spa service license required by Section 727.02 to any applicant who pays a fee of fifty dollars ($50.00) and submits a written application containing the following information:
(a) A complete identification of the applicant and all persons directly or indirectly interested in the permit, and, if the applicant is a corporation, the names, residence and business addresses of the principal officers thereof and of any persons holding twenty percent (20%) or more of its shares of stock outstanding or ownership of the beneficial interest therein or a lien thereon. If the applicant is a partnership the names of all partners, including silent partners, shall be given.
(b) The address of the premises where the health spa is to be operated or the health spa service is to be provided.
(c) The name, residence and business address of the manager or person in charge.
Unless otherwise specifically provided, such license shall be subject to City ordinances applicable thereto and to rules and regulations adopted by the Health Department.

727.04 BOND REQUIRED.
No health spa or health spa service license shall be granted pursuant to this chapter until the applicant therefor has executed and deposited with the Health Department a satisfactory bond in the sum of five hundred thousand dollars ($500,000) in favor of the City, and such bond shall be conditioned so that all prepaid health spa service contracts shall be fully performed by the licensee and conditioned so that the licensee will refund the contract price on prepaid health spa service contracts that the licensee is unable to provide. The failure to maintain a valid bond on file with the Health Department shall be grounds to revoke any license issued.

727.05 EXEMPTIONS.
The requirements of this chapter are not applicable to health spas or health spa services operated or rendered by any public or private nonprofit school, college or university; by the State or any political subdivision thereof; or by any nonprofit religious, ethnic or community organization. 727.99 PENALTY.
Whoever violates any provision of this chapter is guilty of a misdemeanor of the third degree for a first offense and a misdemeanor of the first degree for a subsequent offense.
