(a) Permits required by Section 1367.03 may be obtained by making application therefor to the Building Commissioner upon such forms as he may prescribe. Applications shall include, but are not limited to the following information:
(1) One written application with a plot plan of the lot, premises or parcel attached showing all of the following:
A. The exact location of the proposed antenna, and all other buildings on the subject lot; and
B. A description of the antenna proposed; and
C. The exact location and dimensions of all buildings or structures on the subject lot; and
D. Construction plans, specifications, and elevations of the proposed location; and
E. Sufficient details to show the method of assembly and construction.
(2) The name of the owner or owners of the subject property.
(3) The name of the occupant of the subject property.
(4) The address of the subject property.
(5) The name of the contractor or other person who shall be permitted to construct or erect the proposed antenna.
(b) The Building Commissioner must notify, by certified mail, owners of real estate adjoining the property upon which the proposed antenna is to be erected of his intent to grant the permit at least fifteen days prior to issuing same.
