Each application for such permit shall be in writing, upon a form provided by the Board of Health for that purpose. It shall state the name and address of the applicant and a description of the property whereon or wherein it is proposed to conduct a campground. It shall also contain such other information as the Board of Health may require, and it shall be filed by the applicant. It shall be filed with the Board of Health not less than ten days, nor more than 30 days before the campground is made ready for use, and it shall be accompanied by an inspection fee of $5, for which the Board of Health shall issue a receipt.
