Any person, including an applicant for a massage establishment permit, who engages in the practice of massage, shall file an application for a masseur or masseuse permit with the Commissioner of Assessments and Licenses upon a form provided by the Commissioner of Assessments and Licenses and shall pay a nonrefundable filing fee of fifty dollars ($50.00) for an application to the City Treasurer who shall issue a receipt which shall be attached to the application filed with the Commissioner of Assessments and Licenses.
