(a) Every rental agency shall make an annual application to the Commissioner of Assessments and Licenses for the license required in Section 682A.02. Such application shall be on a form provided by the Commissioner, such form having been reviewed by the Director of the Office of Consumer Affairs or a person designated by him or her for that purpose. The application form shall contain the full name and address of the applicant, the name under which the business is to be conducted, the name of every person with a financial or proprietary interest in the business, the names and residences of the principal officers of the business, the locations where the business is to be carried on and such other information relative to the business as may be deemed necessary by the Commissioner with the advice of the Director of the Office of Consumer Affairs.
(b) Each rental agency must procure a separate license for each location at which business is conducted.
