§ 3141.13 Accidents
   The owner or lessee or person in charge of any of the devices regulated under the provisions of this chapter shall immediately notify the Commissioner of every accident to any person, or damage to the device or to apparatus on or about or in connection with such device involving repairs or replacements exceeding twenty-five dollars ($25.00) in cost, and shall afford the Commissioner every facility for investigating such accident or damage. The Commissioner shall make an investigation immediately thereafter, and shall file in his or her Division a full and complete report of such investigation. Such report shall give in detail all material facts and information available and the cause or causes so far as they can be determined. Such report shall be open to public inspection at all reasonable hours. When an accident involves the failure or destruction of any part of the construction or operating mechanism of such a device, it shall be unlawful to use such device until it has been made safe, and the Commissioner may, if he or she deems it necessary, order the discontinuance of the use of such device until a new Certificate of Operation has been issued by him or her for its use. It shall be unlawful to remove any part of the damaged construction or operating mechanism from the premises until permission to do so has been granted by the Commissioner.
