(a) Issuing of Certificates. A Certificate of Operation shall be issued by the Commissioner when inspections and tests required by Section 3141.11 show that elevators, dumbwaiters, moving stairways, special equipment or amusement devices are installed in accordance with the provisions of OBBC, the Ohio Elevator Code and this Building Code.
(b) Numbering of Elevator Certificates. A Certificate of Operation for an elevator shall show the serial number of the elevator for which it is issued.
(c) Posting of Certificates. The Certificate of Operation shall be suitably framed, under glass, and shall be posted in a conspicuous location in the elevator car, or on, near, or plainly visible from the dumbwaiter, moving stairway, amusement device or special equipment to which it applies.
(d) Limited Certificates.
(1) The Commissioner may permit the temporary use of an elevator, dumbwaiter, or moving stairway for service during the installation or alteration of such equipment under the authority of a Limited Certificate of Operation.
(2) Such Limited Certificate for an elevator shall not be issued until the elevator shall have been tested under contract load, and the car safety and terminal stopping equipment shall have been tested to determine the safety of the equipment for construction purposes, and until permanent or temporary guards of enclosures are placed on the car and around the hoistway and at the landing entrances on each floor. Landing entrance guards shall be provided with locks that can be released from the hoistway side only. Automatic and continuous pressure elevators shall not be placed in temporary operation from the landing push buttons unless door locking devices and/or interlocks required by OBBC, the Ohio Elevator Code and this Building Code are installed and operative.
(e) Life of Limited Certificates. Limited Certificates of Operation shall be issued for a period not to exceed ninety (90) days. Such certificates may be renewed at the discretion of the Commissioner.
(f) Posting of Special Notice. Where a Limited Certificate of Operation is issued, a notice bearing the information that the equipment has not been finally approved shall be conspicuously posted on, near or visible from each entrance to such elevator, dumbwaiter or moving stairway.
