Whenever electrical shock or burns from electrical current result in serious injury or death of any person, the person, firm, or corporation owning or operating the electrical wiring or equipment involved in such accident shall file a report with the Commissioner describing the circumstances and conditions under which such accident occurred. Each certificate of approval of electrical installations shall bear a statement requiring such report.
