1196.03 APPLICATION CONTENTS AND FEES.
   (a)   Nine copies of an application for a Planned District zone change shall contain at least the following information:
      (1)   Name, address and phone number of the applicant;
      (2)   Present use of the property;
      (3)   Present zoning district;
      (4)   Proposed use, including the principal person(s) and/or business establishment(s) involved;
      (5)   Proposed zoning district;
      (6)   Tax map showing property lines, streets, and existing and proposed zoning boundaries of the property;
      (7)   Statement listing the names and addresses of owners of all lots within the area of the proposal and within 300 feet on all sides of any part of the proposal;
      (8)   Development plan (site plan) of the zone change area indicating the location of existing and proposed structures and the proposed use of the structures; parking areas, including traffic circulation and access, pedestrian areas and walks, drainage system based on final grade elevations and landscaping;
      (9)   Schedule or text indicating the total floor areas, by type of use, seating capacities, dwelling units, number of floors and other quantities relevant to the proposal;
      (10)   Schedule or text indicating the total square feet and percentage of land to be occupied by structures and pavement;
      (11)   Drawings or photographs that depict the style, size and type of exterior materials of the structure(s); and
      (12)   Schedule or timetable of proposed construction/occupancy.
   (b)   The Clerk of Council is hereby authorized and directed to require and collect a fee of two hundred fifty dollars ($250.00) for a planned district zone change to pay for the costs of advertising, recording and other costs incurred in the processing of the application. The foregoing fee shall accompany the application and shall be non-refundable. In the event that the actual costs exceed this fee, the Clerk of Council is hereby authorized and directed to require and collect the additional amount needed to pay for all costs.
