(A) No connection shall be made to any sewer or water main nor to any sewer or water lateral, except after the written application therefor has been approved by the city.
(B) (1) Every application for a sewer or water connection shall give the following information:
(a) The name of the owner of the lot;
(b) The name of the street on which the lot is situated;
(c) The number of the house, if there is one on the lot, or, if not, a description of the location of the lot;
(d) The number and kind of connections desired; and
(e) The character of surface of the abutting street.
(2) Every application shall be signed by the person making the application, shall be accompanied by the proper fee for making the connection applied for, and shall be filed with the City Clerk.
(C) Every application for a water or sewer connection shall be approved by the city unless the connection applied for is of a size or is so located that it cannot be made without impairing the city's ordinary sewer and water services.
