(A) Any person desiring to coach an organization sponsored by the city shall make application to the Director of Parks and Recreation.
(B) The application shall include:
(1) Full name, address, date of birth, social security number, driver's license information, race and sex;
(2) A statement acknowledging convictions of any crime, felony or misdemeanor and authorizing a criminal background check; and
(3) A completed applicant fingerprint card.
