1066.03 IDENTIFICATION OF ELIGIBLE PERSONS.
   (a)   No person who has not been issued and who does not display for examination a Newark senior sedan identification card shall ride in a senior sedan vehicle, except those persons who are attending authorized passengers as set forth below. In the event that an eligible person requires assistance above and beyond that which might reasonably be performed by the driver, the rider may be accompanied by a nurse, attendant or side of his own choosing, provided that such nurse, attendant or aide pays the requisite fare.
   (b)   Senior sedan identification cards shall bear a serial number, the name, address and telephone number (if any) of the person to whom the card is issued, as well as the name and telephone number of the person's physician. Such cards will be issued upon application to the Director of Public Service or his designee.
   (c)   In the event of the loss or mutilation of a senior sedan identification card, a new card, bearing a new serial number, will be issued to the holder of the original. card for an additional fee of one dollar ($1. 00). Re-issuance of cards shall be made upon written application to the City by the person and upon verification that the card has been lost or mutilated. All mutilated cards must be returned to the Director for retention or destruction as appropriate. All cards which are found following the issuance of a new card must be surrendered to the City for retention or destruction as appropriate. In any event, the serial number of a lost or mutilated identification card shall be invalid and shall not be used or re-issued.
