A. Fee: Application for such licenses shall be made upon forms provided by the Clerk and each applicant shall pay an annual license fee of thirty five dollars ($35.00) at the time of making such application.
B. Insurance: The applicant shall file with the Clerk evidence of public liability insurance acceptable to the City to be in full force and effect for the period of said license, with minimum limits of one hundred thousand dollars ($100,000.00) for property damage, five hundred thousand dollars ($500,000.00) for a single personal injury or death, and five hundred thousand dollars ($500,000.00) for multiple injuries or deaths, which insurance shall indemnify and hold harmless the City, on account of any and all damage or injury, including death, to real or personal property, whether public or private, which arises out of or is connected with or caused by such tree trimming contractor or his agents in the performance of his business.
