In addition to other information required of all permit applicants under this code, applicants for permits under this chapter shall provide the following information on the application form:
(A) Names and addresses of the owners, lessees and principal operators of the business;
(B) Names and addresses of the owners of the real estate upon or within which the business is conducted;
(C) The size, in square feet, of the premises within which dancing or skating will take place; and
(D) The zoning classification of the property. Proper zoning must be obtained prior to making application for a permit.
In addition to the qualifications set out in § 4-1-1-3, the City Controller may refuse to issue a permit required by this chapter for any of the following reasons:
(A) The application was not made at least 48 hours prior to the time of commencement of the dancing or skating event;
(B) The applicant has been convicted of a violation of law relating to intoxicating liquors, narcotics or disorderly or immoral conduct; or
(C) The applicant permitted violations of law to occur at a prior dancing or skating event held by him or her, without stopping the violations or reporting them to the police.
Dancing and skating events operated under permits issued pursuant to this chapter, as well as dancing or skating schools, shall at all times be open to public authorities of the City for entrance, inspection and enforcement of this code and state law, including regulations set by local fire authorities such as occupancy and other public safety requirements.
