(a) Application for the license required by § 32-32 shall be made in writing to the village manager for each intercity bus drop-off, and shall state the name and address of the owner or applicant, and the names and addresses of the person to operate each vehicle used, and the number of vehicles to be used. Applications for intercity buses must include the information listed in subsection (b).
(b) Applications for intercity busses must include the following information:
(1) The full name, full address, and telephone numbers, including mobile telephone numbers of the owner, operator, and driver of the intercity bus;
(2) The full name, full address, and telephone numbers, including mobile telephone numbers of any entity or individuals that have either directed, paid for, or financed the transport of persons who will disembark in the village;
(3) The make, model, state registration/plate number, fleet number, and vehicle identification number (VIN) of the vehicle that will be providing the service;
(4) The number of people who are proposed to disembark in the village;
(5) The name, address, or location in the village where the applicant proposes to allow passengers to disembark;
(6) The full names, addresses, telephone numbers, including mobile telephone numbers, and electronic mail addresses of all entities or individuals who will be present to meet and receive the passengers disembarking in the village;
(7) A detailed, written plan identifying how the disembarking passengers will be cared for, housed, and fed upon disembarking in the village;
(8) Any additional information that the village manager may reasonably require; and
(9) An affirmation by the applicant that the information provided in the application is true and correct. Such affirmation must be attested to by a person authorized to administer paths or witness affirmations within the state or territory where the application is sworn to or affirmed.
(c) Review of applications for intercity buses. Upon receipt of the application for an intercity bus license submitted pursuant to this section, the village manager will undertake a review of the information contained in the application to verify its truth and authenticity. If the application is incomplete, the village manager will, in writing, advise the applicant of the additional items needed to complete the application. No later than three business days after receiving a completed application, the village manager will inform the applicant, in writing, that the application is approved or denied for the proposed location of disembarkation in the village.
(d) Intercity bus driver obligations. The driver of an intercity bus granted a license pursuant to § 32-33 to stop in the village must ensure passengers remain on the bus until the designated village employee is on-site at the designated location for disembarkation to assist in making sure passengers will be transported to their final destination and cared for upon disembarking in the village.
(e) Proof of application and approval. The driver of an intercity bus granted a license pursuant to § 32-33 to stop in the village must have in its possession a physical or electronic copy of an approved application and present it for inspection at the place of disembarkation, upon request by any employee or designee of the village.
(f) Compliance with approved application. No person may permit the disembarkation of passengers from an intercity bus except in compliance with the application approved pursuant to § 32-33 of this Code.
(g) Revocation. The approval of an application pursuant to § 32-33 may be revoked by the village manager upon the manager's determination that the application contains any false or misleading information.
