Application for a permit to use a mechanical power driven or operated machine as required by § 30-33 shall be made to the village manager, and shall state thereon the size and type of machine to be used, the location of the intended excavation, tunnel or grading, the size thereof, the purpose therefor, the name of the person doing the actual excavating, tunneling or grading and the name of the person for whom the work is being done.
