1369.08 MODIFICATION AND REVOCATION OF APPROVAL.
   (a)   The City Planning Commission may modify, revoke or terminate any previously granted approval of a family home where upon the basis of evidence presented, the Commission determines that there has been noncompliance with the conditions of approval and regulations set forth in this chapter, or that there has been noncompliance with City and/or State codes, or that there has been a failure to maintain the property in conformance with the maintenance plan included in the application filed with the Commissioner of Buildings.
 
   (b)   Any approval of a family home as provided in this chapter shall be automatically revoked or terminated upon the revocation or termination of any license, approval or certification by any County, State or federal agency, or upon the loss of funding for such family home, or upon a change in the identity of the individual, group or agency operating the home and/or, in the case of an individual, termination or revocation of the sponsorship or written agreement or contract with a group or agency not for profit. Where approval of a family home has been automatically terminated or revoked as provided herein, any other individual group or agency seeking to own or operate such home shall apply to the Commission for approval and confirmation of Council as set forth in this chapter. Any family home previously approved by the Commission and confirmed by Council in which there has been a change in ownership or operation may continue in operation pending consideration and approval of an applicant as provided herein. A mere change in supervisory or management personnel or employees of the individual, group or agency operating a family home shall not require revocation or termination of approval and shall not, for the purpose of this section, be considered a change in operation of a family home.
 
   (c)   Any change in admissions policy shall require an automatic review and could be grounds to modify or revoke approval.
