1369.05 APPLICATION.
   Any individual, group or agency intending to establish a family home shall file with the Commissioner of Buildings an application, upon a form provided by the Commissioner, which shall include the following information:
   (a)   The name, address and phone number of the applicant.
   (b)   The name, address and phone number of the applicant's sponsoring group or agency.
   (c)   The address of the home.
   (d)   The projected capacity of the home.
   (e)   The agency which is expected to issue the license, approval or certification to the applicant to operate the home, a copy of the application submitted to such agency and, in the case of an individual, the name and address of the group or agency sponsoring or under written agreement or contract with such individual.
   (f)   Timing of any certificate and license; renewal process and dates.
   (g)   Experience of agency, applicant and supervisors, including references.
   (h)   A maintenance plan for the home and exterior ground and the names and addresses of those persons who will be responsible for the physical maintenance and those persons who will be responsible for providing funds for such maintenance.
   (i)   Projected capital and three-year operating budgets, including revenues and expenditures. These should include projected expenditures for such items as Building Code compliance, improvements, staffing and annual maintenance program.
   (j)   The projected number of automobiles to be used by residents and supervisors of the home.
   (k)   A certificate of compliance or a notice of inspection listing Building and Housing Code violations upon the property issued by the Commissioner or his authorized agent.
   (l)   A description of the supervision to be utilized in the operation of the home.
   (m)   A description of the admissions policy, procedures and committee memberships to be utilized for those persons desiring to reside in the home.
   (n)   Any other information which the Commissioner and/or the City Planning Commission may reasonably require.
