An application shall be completed by the tradesmen and shall contain the following information:
(a) Cleveland license number, if any;
(b) Company or operating name;
(c) Company address;
(d) Federal identification number and/or social security number,
(e) Telephone number;
(f) Signature of president (or other officer of company); and
(g) Type of tradesmen (plumbing, heating, or the like).
