Every person who operates or desires to establish a child day-care home shall apply to the Mayor or to his representative for a permit on such form as he prescribes and as approved by Council. The permit shall be renewed annually, and a fee of thirty-five dollars ($35.00) shall be paid for each permit or renewal thereof. Fees collected under this Section shall be paid into the City's General Fund.
Upon filing, the application shall be forwarded to the Zoning Administrator for review, the Police Department for completion of background check, to the Fire Department for inspection of the premises under Ohio Fire Code, Life Safety Code and Existing Structure Code including this Chapter 767 and all other Codes of the City of Garfield Heights. When, after the provisions set forth in this Chapter have been completed, and the Mayor or his representative is satisfied that all requirements are complied with, a permit shall be issued in such form and manner as he prescribes but including at least the name of the property owner and the name of the provider. The permit shall be valid only for the provider at the address designated on the permit and the permit shall be posted in a conspicuous place at the subject premises. The permit shall not be transferable.
