758.05 APPLICATION.
   (a)   Written application shall be made to the Service Director on a form furnished by him. The applicant shall furnish on the form, the make, model and license plate of each motor vehicle to be used and the name, address and telephone number of the owner and/or operator employed, including subcontractors. Proof of liability insurance must be attached to application form.
   (b)   A decal shall be issued to each contractor for each vehicle employed in snow removal and the decal shall be prominently displayed on each vehicle. A certificate of registration shall be issued to each operator which shall evidence compliance with the provisions of this chapter. The City may refuse the issuance of any permit or revoke a previously issued permit for good cause shown. (Ord. 76-2010. Passed 11-22-10.)
