(a) Each applicant registered under the provisions herein shall be at least eighteen years of age and the holder of a valid Ohio driver's license permitting him to operate snow removal vehicles.
(b) Each operator regardless of whether he is employed by a firm or corporation registering under the provisions herein, shall be listed separately under the firm's heading and shall be qualified individually for registration.
(c) The names of all persons employed by the applicant, whether by firm, corporation or subcontractor, who are authorized to use permits in the name of the applicant must be listed on the Snow Removal Registration Form and must be covered by the applicant's insurance liability policy.
