Application for the license shall be made to the Finance Director upon such blank forms as shall be prescribed therefor. On such application the applicant shall state his name, whether an individual, partnership or corporation, and if a partnership, the names and addresses of the partners composing the same, the addresses at which it is desired to maintain and operate the mechanical musical instrument, the nature of the business conducted at such place, the make, name, model, serial number and other identifying information with reference to the particular mechanical musical instrument which it is desired to operate, the name and address of the owner of such instrument, and such other and further information as the Finance Director may require.
