(a) No person being the owner of an establishment or amusement center shall display for public patronage or keep for operation by the public, any mechanical or electrical amusement device without first complying with Section 1141.03 of the Planning and Zoning Code and obtaining a license issued by the Building Commissioner pursuant to this chapter.
(b) No person being the owner of an establishment or amusement center shall display for public patronage or keep for operation by the public, any mechanical or electrical amusement device without first obtaining a license issued by the Commissioner pursuant to this chapter.
(c) Each owner of a business, establishment or amusement center subject to this chapter shall, in addition to the license fee delineated in this section, pay any additional fees provided in Section 701.07 per year for each electrical, electronic or mechanical amusement device located on such premises.
(d) No transfer of a license, either as to person or place, shall be permitted except with the written consent of the Commissioner, which consent shall be endorsed upon the license.
(e) No license shall be issued until an application is submitted to the Commissioner, which application shall, among other things, state:
(1) The name and address, age and birth date of the applicant;
(2) The location where the mechanical or electrical amusement devices are to be displayed or operated and the business conducted therein;
(3) A description of the machine or machines to be covered by the license, its mechanical features, the name of the manufacturer and the serial number of the machine;
(4) The name of the owner or owners of the premises wherein the devices are to be displayed or operated;
(5) The name of the owner or owners of the business conducted therein;
(6) The name of the owner of the machine, if the person is different from the applicant; and
(7) Whether or not the applicant or the owner of the machines has been convicted of a gambling offense of for the sale or conviction of a drug offense in the last ten years prior to the date of application.
