1313.03 LICENSE APPLICATION; FEES.
   Persons desiring a license as required under Section 1313.02 shall file an application for the license with the Chief Building Official or his representative on a form provided by him. An application shall be made to the Chief Building Official or his representative by the applicant.
   (a)   The Chief Building Official or his representative is directed to have the applicant show proof of license from another city as to provide a basis for judgment as to whether the person applying has an average competence to perform the services of his trade.
   (b)   The applying contractor has completed the application form to the satisfaction of the Chief Building Official.
   (c)   The applying contractor has furnished evidence of liability insurance for bodily injury in the minimum amount of five hundred thousand dollars ($500,000) and for property damage in the minimum amount of two hundred thousand dollars ($200,000).
   (d)   The applying contractor has tendered the certificate of license fee as specified in Section 1301.01(d)(8).
   (e)   Furnish a performance bond in the amount of ten thousand dollars ($10,000).
   (f)   Must complete an application for certificate of tax registration.
