1198.02 APPLICATION FOR CONDITIONAL USE PERMIT.
   An application for a conditional use permit shall be filed with the Chairman of the Board of Zoning Appeals by at least one owner or his designated agent of property for which such conditional use is proposed. A fee of seventy-five dollars ($75.00) shall be paid to the Zoning Inspector at the time a conditional use permit is requested, which the Zoning Inspector shall forthwith pay to the Auditor's Office to the credit of the City. At a minimum, the application shall contain the following information:
   (a)   The name, address and phone number of the applicant;
   (b)   The legal description of the property;   
   (c)   A description of the existing use;
   (d)   The present zoning district;
   (e)   A description of the proposed conditional use;
   (f)   A plan of the proposed site for the conditional use showing the location of all buildings, parking and loading areas, traffic access and traffic circulation, open spaces, landscaping, refuse and service areas, utilities, signs, yards and such other information as the Board may require to determine if the proposed conditional use meets the intent and requirements of this Zoning Ordinance;
   (g)   A narrative statement evaluating the effects of such elements as noise, glare, odor, fumes and vibration on adjoining property; a discussion of the general compatibility with adjacent and other properties in the district; and the relationship of the proposed use to the comprehensive plan;
   (h)   Such other information as may be required in Section 1197.05.
