(a) Applications for permits shall be made in writing on forms prescribed by the City, and shall state the name of the person, firm, corporation or other entity which is to do such electric work, name and address of the owner or owners of the building or property in which such work is to be done, and when such work is likely to be commenced.
(b) After inspection is made of the wiring work done in the City, no alterations may be made in such wiring without first notifying the City and securing a permit, except for minor work such as repairing flush and snap switches, replacing fuses, changing lamp sockets and receptacles, insulating bare joints and repairing drop cords.
(c) Pursuant to the Ohio Building Code, the Chief Building Official or his designee may request the applicant to furnish a plan and specification completely describing the work to be installed before a permit is issued. Such plan and specification shall remain on file with the City. Any contractor or homeowner who fails to obtain permits required by this chapter prior to the beginning of work, except in those cases where the emergency installation of electric is necessary, shall be required to obtain and pay for the permits which were originally required and, in addition thereto, shall pay a penalty twice the amount of the original permit fees.
(d) Substitution. The Electrical Inspector may issue a second permit for an electrical installation (rough or finish) upon receipt from the property owner, his authorized representative or a project general contractor, of a written signed statement advising the City that the original permit holder has been removed from the electrical installation job and further identifying the licensed electrical contractor who has been retained to complete the electrical installation. Issuance of a second permit shall be subject to all of the requirements of this chapter, to include payment of permit fees, and shall be further subject to the requirement that the second permit holder shall be responsible for the completion of the electrical installation, to include any work initiated by the first permit holder, in accordance with the requirements of this chapter.
(e) Expiration. An electrical permit shall be valid for a period of twelve months from date of issuance.
(f) Revocation. Any electrical permit found by the Electrical Inspector to have been obtained by misrepresentation of facts or subterfuge shall be subject to revocation by the Electrical Inspector.
