(a) Application for all permits shall be made in writing by the owner, or his agent, upon forms furnished by the City. Such application shall be accompanied by triplicate copies of the plans and specifications required by the Ohio Building Code. The application shall include the following:
(1) Calculation of square footage per floor;
(2) Use Group as defined by the Ohio Building Code;
(3) Construction Classification as defined by the Ohio Building Code; and
(4) Description of land, including lot number, address and name of owner.
(b) All applications and plans and specifications shall be dated upon receipt in the office of the Chief Building Official, and taken up in their order as received, unless buildings or structures are of a complex character which require prolonged examination and inspection.
(c) No person shall give or furnish false information in the application for a permit.

(d) All applications for permits shall include proof to the satisfaction of the Chief Building Official of the following:
(1) For new construction and for renovations or repairs to existing structures, the applicant will provide adequate arrangements for the orderly collection and removal of construction debris and other solid waste generated at the work site; and
(2) For new construction, the construction site will have adequate facilities for temporary sanitation needs.
