151.12 CONTINUING DISABILITY BENEFITS.
   (a)   A full-time employee who exhausts all accrued sick leave may receive continuing disability benefits for non-occupational disabilities as follows:
      (1)   The employee shall apply according to administrative regulations established by Human Resources. The application must include medical verification.
      (2)   There is a five (5)-day waiting period before receiving continuing disability benefits.
      (3)   The City may deny the application of an employee who has been disciplined for absenteeism or abusing sick leave within twelve (12) months of the application.
         
   (b)   If the appointing authority grants the application, the City shall pay the employee at sixty percent (60%) of their regular rate of pay as follows:

   (c)   The maximum benefit an employee may receive during their employment with the City is thirty (30) weeks. For example, an employee with more than fifteen (15) years of service and who previously used four (4) weeks is only eligible to receive an additional twenty-six (26) weeks of disability.
   (d)   An employee on disability leave receives medical, prescription, vision, dental, and life insurance. The employee does not accrue sick leave, vacation, or longevity.
