705.04 APPLICATION FOR ALARM SYSTEM BUSINESS LICENSE; ISSUANCE OF LICENSE.
   (a)    Any person desiring to operate an alarm system business in the City shall make application for a license to the Director of Public Safety on such forms as the Director may prescribe, which shall include but not be limited to the following information:
      (1)    The name, address and telephone number of the alarm business and the type of business entity it is (individual, partnership or corporation).
      (2)    The name, address and telephone number of the individual proprietor, partners or directors and principal officers, depending on the nature of the entity applying.
      (3)    A description of the alarm systems and devices offered for sale or lease to the public and a description of any services related to alarm devices offered to the public.
      (4)    A list of all sales agents, representatives or employees of the business.
      (5)    A list of all felony and misdemeanor convictions of individual proprietors, partners, directors, officers, sales agents, representatives or employees of the applicant's business. The date and location of each conviction shall also be included.
      (6)    A statement as to whether the applicant has ever been denied a license or permit in any jurisdiction to engage in the alarm business or has had such license or permit revoked.
      (7)    A statement as to the length of time the applicant has been engaged in the alarm business and where engaged.
      (8)    A statement that the applicant agrees to update the above information upon application for renewal of alarm license at least every year as provided below.
   (b)   The license fee for each alarm system business shall be one hundred fifty dollars ($150.00). The license issued shall permit its holder to operate the licensed business under the terms and provisions of this chapter for a period of one year beginning on the first day of July of the year of issuance, unless previously revoked with cause by the Director of Public Safety. License fees shall not be provided for partial years.

   (c)   Persons already engaged in the alarm system business in the City on the effective date of this section shall submit an application for a license no later than thirty days from the effective date of this section. Such applicant may continue to do business while the license application is being processed. An applicant not previously engaged in providing services as an alarm business in the City on the effective date of this section shall not commence doing business until the application is approved.
