The Director of Public Safety may issue a license to the owner to operate an ambulance if the following requirements are met:
(a) The application form supplied by the Director of Public Safety shall be completed, sworn to, signed by the authorized applicant and shall have attached two copies of a dated and current list of charges. An updated list shall be forwarded to the Director semiannually or upon any change in pricing, whichever occurs first.

(b) The applicant shall garage and staff and have available at least one licensed ambulance twenty-four hours a day, seven days a week. The garage shall be heated to at least sixty-five degrees and have sanitary facilities.
(c) No person shall submit more than one application form for each ambulance or advertise under any name other than the name in which the license is issued.
(d) Each ambulance shall be equipped with a siren, emergency flashing lights, trade name of business and star of life affixed on each side of the vehicle, and such other reasonable markings as may be prescribed by the Director of Public Safety.
(e) While in service, ambulances and equipment ordered after January 1, 1984, shall comply with the equipment and operational standards established by the State Emergency Medical Service Advisory Council Manual, a current copy of which shall be maintained on file at the Fire Department Headquarters for inspection and copying by the public.
(f) Each vehicle, its equipment and its garage shall be inspected by the Director of Public Safety or his authorized representative before issuing a license, to determine compliance with such safety and health requirements as may be set by the Director. (Ord. 269-83. Passed 11-21-83.)
(g) The applicant shall submit to the Director, a signed certificate of insurance, complying with the insurance requirements of Section 703.08.

(h) The applicant shall disclose the individual names of owners, managers and controllers of the ambulance business
(i) The applicant, for an ambulance license, shall furnish proof that he shall have sufficient licensed drivers and attendants to properly staff and operate each ambulance proposed to be used. (Ord. 269-83. Passed 11-21-83.)
(j) If satisfied as to the safety of the vehicle, the equipment provided and the character of the applicant, pursuant to the regulations of this chapter, the Director shall grant the license. (Ord. 39-91. Passed 3-11-91.)
