Every person driving a taxicab must be licensed as such. Each applicant for a driver's license must:
(a) Be twenty-one years (21 yrs.) of age or over, an American citizen, or have declared his intention to become such a citizen, and a bona fide resident of Cuyahoga County for thirty (30) days next preceding date of such application;
(b) Be of sound physique, with good eyesight and not subject to epilepsy, vertigo, heart trouble or any other infirmity of body or mind which might render him unfit for the safe operation of a public vehicle;
(c) Be able to read the English language, to the extent necessary to read directions and traffic signs;
(d) Be clean in dress and person and not be addicted to the use of intoxicating liquors or drugs;
(e) Produce, on forms to be provided by the City Manager, two (2) character references of persons not related to the applicant;
(f) Fill out, upon a blank form to be provided by the City Manager, a statement giving his full name, address, age, color, height, color of eyes and hair, place of birth, length of time he has resided in the City, whether a citizen of the United States, places of previous employment, whether he has ever been arrested or convicted of a felony or misdemeanor, whether he has been summoned to court, whether he has previously been licensed as a driver or chauffeur and if so, whether his license has ever been suspended or revoked and for what cause, which statement shall be signed and sworn to by the applicant and filed with the City Manager as a permanent record. Any false statement make by the applicant for a license shall be promptly reported by the City Manager to the prosecuting attorney on Cuyahoga County. The City Manager is authorized and empowered to establish such additional rules and regulation, covering the issuance of a driver's license, not inconsistent herewith, as may be necessary and reasonable.
