1744.15 DISCHARGE POLICIES.
   (a)   A group home shall maintain a permanent written record of discharge on each resident who leaves the facility. The permanent written record of discharge shall include: date of discharge; the reason for discharge; the disposition of money and valuables held for safekeeping; and a forwarding address of the resident, a designated relative, the legal guardian or a friend.
   (b)   A group home shall notify the resident and the agency responsible for the placement in writing, at least thirty days in advance, of the facility's intention to remove the resident from the facility. A resident may be discharged with less than thirty days' notice if the circumstances pose a threat to the health, safety or well-being of the resident or others. The resident shall be entitled to discharge rights as outlined in the Ohio Administrative Code Section 3701-20-24.
