1744.14 UNUSUAL INCIDENT REPORTS.
   An unusual incident report shall be prepared for each incident occurring in a group home. An unusual incident report shall include the following information:
   (a)   Names of persons involved in the incident;
   (b)   Date, time and place of the incident;
   (c)   A statement regarding the probable cause of the incident;
   (d)   A narrative description of pertinent events of the incident;
   (e)   A narrative description of corrective measures taken to avoid repetition of the incident.
   A copy of all unusual incident reports shall be forwarded to the Health Commissioner within seventy-two hours of the incident and shall be kept at the group home in a special unusual incident report file for a period of time no less than five years.
