§ 31.026 CONFLICT OF INTEREST DISCLOSURE STATEMENTS.
   (A)   The disclosure statement required in § 31.016 of this chapter shall be on a form (Exhibit A attached to the ordinance codified herein) obtained from and filed with the City Clerk and shall be signed as a verified statement. Any violation of this section shall be subject to all remedies and punishments as may be provided for under state law, Charter or ordinance, including, but not limited to, forfeiture of office removal for cause. Public officials serving at the time of adoption of this subchapter shall file the conflict of interest disclosure statement within 14 days of the date said subchapter takes effect.
   (B)   The form shall include the following information:
      (1)   Date, month, year;
      (2)   Name of person filing;
      (3)   Address of person filing;
      (4)   Name of person or business entity with which a conflict exists;
      (5)   Address of person or business entity with which a conflict exists; and
      (6)   Does said person or business entity do business with the city or with an elected official or appointee in a policy making position and, if so, the amount of income received or anticipated.
